What is the Connecting Communities Learning Forum format?

The format of the Connecting Communities Learning Forum will be highly interactive; participants will have many opportunities to talk with national experts in HIE and their peers, including representatives from communities that have successfully shifted away from paper record keeping. The Connecting Communities Learning Forum is a unique opportunity for practitioners and leaders in these collaboratives to identify common principles to advance their work, as well as strategies to tackle common issues--clinical, financial, legal, organizational, and technical.

The theme of the Connecting Communities Learning Forum will be eHealth Initiative's recently launched Connecting Communities Toolkit that is a structured, how-to synthesis of principles and tools designed to equip states, regions and local communities with the information and expertise to begin or advance health information exchange initiatives and organizations to drive improvements in healthcare. The Connecting Communities Toolkit offers insight into areas crucial to start-up and successful survival such as organization and governance, value creation and financing, practice transformation and quality, information-sharing policies, technical aspects and public policy and advocacy.

 Registration

How do I register as a Presenter?

If you are presenting at the CCBH Learning Forum you will receive a complimentary pass to attend.

How do I register as a regular attendee?

Please submit the online registration form or the downloadable PDF form on this website. 

Is there a special price if I’m employed by a community, regional health information exchange and/or government agency directly involved in health information exchange?

Individuals employed by communities, regional health information organizations, and government agencies directly involved in health information exchange pay $295.

Are there any discounts for multiple attendees?

There is an aggregate price to 3 individuals from the same organization for $495. The aggregate group must be employed by communities, regional health information organizations, and government agencies directly involved in health information exchange (this registration option is only available via fax or mail registration) 

Is it possible to obtain a “one day only” pass?

No, only full conference passes are available.

Cancellation

What is the cancellation policy?

No refunds will be given for "no-shows" or for cancellations. You may send a substitute; please call the Conference Office at 1-800-684-4549.
