The Connecting Communities Toolkit offers users a comprehensive look into the progress of health information exchanges (HIEs) in communities across the country.

In July 2004, the Foundation for eHealth Initiative announced the award of contracts to help communities evaluate important aspects of their projects and support the dissemination of these lessons as broadly as possible.  Details of the Foundation's selection process are available in the "Funded Communities" section, accessible in the navigation bar to the left.

The "Community Profiles" section offers an overview of communities that are pioneering the development and advance of HIEs across the country.  Each of these nationally-recognized efforts have agreed to share information and tools for others seeking guidance to developing their own HIEs.  To learn more about these leading communities and links to valuable resources, click on the "Community Profiles" section, accessible in the navigation bar on the left.

Finally, the "Community Directory" section provides a unique repository of information from 134 HIEs organized by states.  This inventory houses a wealth of information on each effort including project summaries, lists of participating partners, and the name and contact information for the official point of contact.  In the coming months, this directory will be enhanced to allow existing communities to update their profiles, and new users the opportunity to add information about their efforts.
