The Pacific Northwest Connecting Community will focus efforts on creating mechanisms for developing home-centered coordinated care systems for people diagnosed with chronic disease.  The PNCC will explore options for tracking signs and symptoms experienced by people with chronic disease in order to provide optimum case management by their healthcare providers while potentially reducing the necessity for unscheduled visits that often cost the patient in time and energy while additionally taxing the system's resources.  Our initial team is comprised of over twelve individuals who meet frequently to discuss these issues and possible apporaches.  Our first systems will focus on lung cancer.
